The Director General of Space Science Agency Dr. Venom was exiled to Planet Sard for a failed coup d'état. In the year 6665, he escapes and invades Planet Nemesis and the seven planets it controls with the help of Bacterion. The Nemesis High Council sends James Burton, ex-pilot of the Vic Viper, to pilot Metalion and attack Dr. Venom and the Bacterion invaders. The game takes place during the year 6666.

It was the first MSX game with an SCC sound chip.
Gradius 2 was a bilingual cartridge. As Konami was an international company with worldwide sales and roots in Japan, they needed to produce two versions of their programs: one for the Japanese market and an English version for the rest of the world. For some games, Konami decided not to produce two different cartridges for different language versions of a game, but put both language versions on one cartridge. This was a possible because the MSX computer BIOS rom contains a region code. Using that code as identifier, the game shows either the English or Japanese version.
In the original manual can be found a contest, registrations were valid from September 15 to November 15 1988. You should take a picture of the Game Over screen and send it to Konami. The Game Over screen contains a checksum of the scoreboard. Here are the lots you can win:
First place: unprecedented Gradius 2 Director's Chair, with his name inscribed.
Second place: Gradius 2 unpublished jacket.
Third place: Gradius 2 unpublished backpack.
The top 2000 players will win a card with their name on it.

A known prototype version contains one extra stage (stage 6) and also a different stage (stage 4). The hidden bonus stages do not seem to be present. The
Gradius 2 contains unlockables by inserting certain Konami game cartridges in the second cartridge slot:
- Inserting Penguin Adventures in the second cartridge slot turns the normal Vic Viper spaceship into a penguin, powerups turn into fishes and blue powerups turn into birds.
- Inserting Knightmare 2 allows you to restore your power-ups after you loose a life.
- Inserting Q-Bert allows you to enter some passwords, press
-- METALION: Your ship turns green and you're immortal. it wears of after a while; just re-enter the password.
-- NEMESIS: You go to the next stage.
-- LARS18TH: You get full power up (two options, laser etc.) and no speed ups.
When you've finished stage 7, you have to go back to stage 1 again (without skipping the stages in between!) and then you'll get the last stage, also known as stage 0.
