You will start a new game from the selected level with 240 hit-points instead of 8, so technically you will be invulnerable for some time. But you can't advance to the next stage playing this mode, you can play only the selected level again and again. If you press A + B at the title screen 6 times, you'll be able to see the ending. But the staff roll will be different from one you should see if you beat the game normally, containing additional programmer's message.
